Two custom fields are configured in Harbor: a Delivery Instructions text field and a How Did You Hear About Us dropdown, both in the Order section of the block checkout. To demo: add a product to the cart, open checkout, fill both fields, place the order, then open it in admin. The values appear after the billing address and in the confirmation email.
